Case Management

Formerly known as SBK Case Management until mid-2022, we are a group of trauma-informed Case Managers providing a unique service: supporting the rehabilitation journey while also acknowledging the distress the injury has caused.

This approach can help clients push through the barriers and feel able to find greater fulfilment in their ‘new normal’.

Many of our clients have significant and complex disabilities, particularly as a result of an acquired brain injury, and require specialist rehabilitation, therapeutic care and support. We understand the impacts of injury, and support our clients towards greater functional independence, wellbeing and quality of life.

We use the CQC framework to follow a communications style around an assessment-formulation-planning-implementing-reflecting process. So, we understand there’s work to be done while there are emotional reactions are to be experienced. Our input can therefore include:

  • Recognising the grief journey
  • Supporting the relationships within the professional networks
  • Supporting to recruit a rehabilitation package of support workers and therapists that are trained to accept the emotional reactions within their roles
  • Supporting to access statutory services including arranging benefits
  • Supporting to plan a holiday
  • Support to liaise with your employer or educational setting
  • Implementing and managing a therapy package
  • Supporting with sourcing suitable accommodation
  • Support to acquire specialist equipment or housing adaptations
  • Providing a voice for clients
  • Support with budgeting

Our team has awareness of and, in some cases, experience of the challenges of living in a country that is different to where they grew up. Understanding the challenges of migration needs to be a relatable cultural experience, personally or theoretically, in order to support minority clients. Our service can add value to the support available to a client because of our unique position as a service with minority sensitivity and representation.
