Care Plan Packages

Care plans are legal documents, and the authorising professional is accountable for the care they have assessed, planned, and are implementing, as well as for upholding professional standards such as record keeping and code of conduct.  They require a thorough understanding of the client's needs and where to best place support.

 

Care plans must be holistic covering different domains of client functioning from mental and physical health to social care needs and cultural requirements, where appropriate.

 

We at SBKCM understand that care plans are an essential and necessary tool in ensuring the health, safety and welfare of our vulnerable clients and families. We also understand that a good care plan takes a lot of time, effort and dedication which can be challenging to achieve well amongst the many other case management tasks.
